• Intervention Matching – Individuals are automatically channeled to interventions depending on the timing of their disease progression and the nature of your existing intervention programs;
• Intervention Tracking – All intervention activities are tracked including interaction with members and their affiliates, outreach program eligibility, assessment and interventions performed, etc.; and,
• Outcomes Tracking - Individual outcomes are tracked and monitored either analytically and/or through configurable surveys allowing you to evaluate non-process oriented outcomes measures.
